We've included some sample text here to get you started.. Please note, do not copy and paste these fields... Always use the form field wizard from the insert menu or toolbar. The trick to inserting new fields within this default layout, is to use the table insert row (right click, Row, Insert Row after ). Once you have created your row space, then you can insert your form field using the prompt options. You must create each one individually and give each field a unique field name.. Use the INSERT... dropdown in the menu above to add more form fields to this page. The text you are reading here is editable.. You can select this sample text and questions with your mouse and delete if you like. :) Use the HTML mode if you need to change the field names or enter additional combo box options.

It is incredibly important that you use the insert wizard to create your form fields. Do not copy and paste form fields, as you will only end up with confusion. Form fields do not know what prompt they sit next to, so you need to explicity give every form field a value. Currently we don't have a "click to edit" form field editor, sorry about that.
THere is a tab at the top called validation, and that is where you can add data validation. You need to take special note of your form field names. You might find it easier to test your form one time and answer all questions on the page, then you will see the field names in the validation page.
